Each member who completes the survey and includes her contact information at the bottom of the form will be eligible for a grand prize drawing. Twenty lucky winners will be randomly selected to receive a $20 gift card to Herff Jones, the official jeweler of Alpha Chi Omega.
The survey consists of 35 questions and will take approximately 10-15 minutes to complete.
